Hello and thanks for notifying us about the problem you had with our product. We fully stand behind our products and will make this unfortunate incident up to you!

WE'LL SEND YOU A LOT OF FREE PRODUCT TO MAKE UP FOR ANY INCONVENIENCE AND SHOW YOU THAT WE TRULY CARE ABOUT OUR PRODUCTS AND STAND BEHIND THEM 100%!

We really will gladly send you some replacement product - but we need some information first.

For legal reasons we have to be sure you are of legal age for tobacco smoking. Our lawyers ask that we get a copy of your drivers license or other official document that shows your date of birth. We know this seems harsh but it's just that our tobacco lawyers want to be sure we don't send tobacco products to someone who is underage. We know that many people don't want to give out this information and we don't want to cause you any grief so please follow the steps on this form. We're just trying to protect both ourselves and underage kids from having access to our products (meaning we don't want anyone underage to have access to any of our products under any circumstances).

We can only send product to the address as it appears on your document (again because of underage legal protections).
